Derby left it late to see off 10-man Accrington with a 1-0 victory in their FA Cup fourth-round tie at the Wham! Stadium.
Martyn Waghorn poked home from close range with 12 minutes remaining after Accrington had looked good value for a replay at the home of the Championship side.
Earlier in the second half, Daniel Barlaser saw a goalbound effort blocked superbly by Derby's Fikayo Tomori, but just moments later the midfielder was sent off for a second yellow card (59).
Derby had Jayden Bogle sent off themselves for a professional foul late on, but, following a frantic last few minutes which saw goalkeeper Kelle Roos save brilliantly from Billy Kee's free-kick, the Rams survived. The draw for the fifth round will take place on Monday evening at 7.15pm.
Having beaten Premier League Southampton in the third round, four-time FA Cup winner Frank Lampard saw his Derby side evenly matched in the first half.
Accrington, who beat Championship side Ipswich in the third round, should have been ahead on 10 minutes but Jordan Clark miscued at the far post unmarked from Sean McConville's fine right-wing ball.
That came moments after Waghorn had seen a header saved well by Jonathan Maxted from Scott Malone's whipped cross, and the remainder of the first half was fiercely contested, if not slightly hampered by poor pitch conditions and a strong wind.
Waghorn came close again after the break, sliding in but failing to meet Malone's cross by a matter of inches, while at the other end Barlaser's fine one-two with McConville saw him free inside the box, only to see his left-footed effort kept out brilliantly by on loan Chelsea man Tomori.
Barlaser, who was booked in the first few minutes, was shown a needless second yellow just before the hour mark after he flew in on Waghorn to leave Accrington with 10 men.
Kee then flashed an effort inches wide as Accrington continued to press, but Derby got their winner in scrappy circumstances as Waghorn stretched to finish off from close range after good work from Duane Holmes inside the box from a well-worked corner.
There was plenty of drama in the final few moments as Bogle saw red for a cynical tug on sub Paul Smyth just outside the box as he ran through on goal, and from the resulting free-kick Roos got down well to his left to tip away Kee's clever free-kick.
